Incident Report – Meeting Incident Involving Timex Llanza (August 22, 2026)

Dear Sir Jorge,

Good day.

I am writing to formally document the incident that occurred during our staff meeting at Holy Angels Memorial Park earlier today.

Chronology of Events:

1. At approximately 9:00 AM, Sir Jorge Saguinsin arrived at Holy Angels Memorial Park. A meeting was subsequently conducted with all GT, ISG, and Office Staff present.

2. The meeting began in an orderly and professional manner. During the middle of the meeting, Sir Jorge calmly asked Timex Llanza whether he had previously borrowed money from Ma’am Elvira.

3. Timex initially answered that he had not borrowed money from Ma’am Elvira. However, based on what was known, he had previously borrowed money from Ma’am Elvira during a time when he was in urgent need of financial assistance.

4. Upon hearing his response, Sir Jorge addressed the matter and became upset because he considered the response to be inaccurate.

5. Sir Jorge asked Timex about the matter again. Timex then acknowledged that he had indeed borrowed money from Ma’am Elvira. During this exchange, Timex’s voice became somewhat raised while responding to Sir Jorge.

6. Due to what Sir Jorge perceived as inappropriate behavior and manner of responding during the discussion, he instructed Timex to leave the meeting.

7. After Timex left, the meeting continued with the remaining staff. The meeting proceeded in an orderly manner and was completed without further incident.

8. Despite the incident, the meeting provided the staff with valuable information and new knowledge that were relevant and beneficial to our work.

This report is being submitted to formally document the sequence of events as they occurred during the meeting.
